Medical History Empty Medical History

I asked this question on the old board, but I think it got lost.

At the beginning of each time I have each student complete a medical questionnaire to get an feel for their general state of health as well as advance warning about any issues they have.

I also record any problems they they have exhibited during class and, if I have suggested that they see a quack and they don't, I record the fact and insist that they countersign the note.

I also discovered, just before the beginning of this term that I would be breaking the law if a student collapsed for any reason and I passed their questionnaire on to the emergency services, so I've had to add a release permission block to the questionnaire.

Does aqny one else do anything like this?

I really wasnt aware of this either, and yet we take the details for future use to help in these situs, what did you put to protect yourself, just to make sure I am covered also?

hi
we do a kinda questionare , but we do a induction now for all new members .
and they get a support card.
a reveiw every six months

they get a membership pack with

medical questionare
photo consent form
licence info
direct debit form
goals

the students like it , it also gives a professional front, and shows you are interested .

Just above the signature block I have the following:

I do/do not (delete as applicable) give permission for the above information to be released to the emergency services in the case of a medical emergency.

we do a lot of promotions with our local papers (try to get in one of them at least once a month )

i'll give you a good way to double your student base ( we have 173 at 1 venue alone) .
go to your local police , offer a free ( yes free) self defence course because of the growing local street and domestic violence (1 hour per week for 6 weeks)
and ask for indorsement .
you will get it and they will do a press release and give you backing .
at leasy 50% that attend will sign up.
do this every 3 months , alternateing between kids ( bully awareness) and adults ( street sense)
in 1 year you will more than double your student base easy.

we are aiming for 300 in the new venue (over half way in 3 months)

any thing mate , whatever your speciality is , find a cause and jump on the band wagoon mate .
obesity is a major problem ( fat burners), stress is a major killer ( stress busters).
it sound cheesey but it works mate .
my base club running at maximum ( should be by april ) will take over £12,000 a month , thats 1 club.and i teach alone ( exept my girlfrien takes the tot's)
even do a sponored exrsise ( form) for a local charity .
if you do anything , even fart mate , tell the papers , they will print anything and it will reach more people than any flyer , and it's free .
the 4 weeks running upto christmas i was in every paper for 4 weeks for 1 reason or another .
here's a good one , phone your local paper and offer to do a piece on the sword ban ( from a martial artist point of view) and as a respected member of your comunity , just make sure you put your website and phone number at the bottom.
i just did it , got me 2 pages.
just some ideas ,
i'm just about to do another series for martial arts illustrated ( did'nt write over christmas) doing a section on low kicks , point of origin strikeing and pinn's and checks , should last most of the year .
then i'll go and tell my local paper and they will do a piece on me being in mai ( local man makes it big ect) , i'll get 20/30 students out of that.

sorry if that sounds callous but it is advertiseing , it does'nt affect my students or the quality of arts taught , just think of it like this ( it's how i justify it)
i want to have the best dojo , for me and my students , the best equipt and nicest enviroment , that takes money .
i will do what ever it takes to get that , it is my responsibilty to my students .

Some good things to think about there.

And right now I need more students. For one reason or another I've lost > 50% of my long term students since the summer.

I'll definitely be giving this some thought.

just looked at your web site mate .
my angle would be this

asertiveness classes .
60% of adults are bullied in the workplace or home.
i'd try this ( just a suggestion mate)

build your self esteme / residual self image/ confidence and assertiveness .
in a freindly relaxed enviroment , go home feeling a new person , with new goals and lease of life to combat this new year .
that resalution can be yours .
free 4 week assertion course , no strings attached .

then go to your local papers , and use a slightly different angle with each one.
like :
in our area domestic violence is up 75% , we're (you) trying to help combat the growing plight of local men and women.
or
street violence is up 55% , we are here to help people , the biggest factor in street crime is confidence and assertiveness , most attackers are looking for a victim .

stuff like that , they love it , and it gets people to now who and where you are , also raises your profile in the local comunity .
most of all it gets people thru the door , for free , your free class would only need be 45 minutes , once a week for 4 weeks .
thats 3 hours of your time .
put the classes before or after one of your existing classes .
get the new (free) students to come and watch ( they usually stay to see what goes on.)

may be worth a try mate .
